Care manager must have zero restrictions, and/or disciplines on their license.

- This is not a full-time position. This is referral based and it tends to fluctuate so this is ideal for someone looking for part-time/supplementary work.

- Our area of need currently is an urgent need area. Within a one hour drive mile radius, you can accumulate up to 1-3 referrals a month.
- State where coverage is needed: Ohio.
- We reimburse $35.00 per visit only. We provide a flat rate of $10.00/visit for mileage. Total per visit = $45.00. Payroll is scheduled for the 15th and 30th if the month after completing a 30-day complete cycle for patient visits and requires complete documentation of our Electronic Medical Record.
- Training is performed online in 2 parts: modules online and a live webinar. All together it’s 6 hours and an additional 2 hours of Ethics. You do NOT have to take it all at once. The Ethics is a mandatory annual training for anyone working with Step of Faith Health and Wellness, Inc. and it’s associating companies/departments.
Position Responsibilities:
· In-home visits with the patient.
· Hospital/facility visits when authorized.
· Facilitating conference calls between the member, the physician and the Care Manager as
needed to clarify treatment plans, medication regimens or other urgent issues.
· Reconciling prescribed medication
· Assessing members’ environment, functional, psycho-social status and financial well-being
· Identifying and developing action plans to empower members to remain and thrive at home
· Connecting members and their families with approved resources, services and professional
intervention within Humana and in their community who can help them address medical,
legal, housing, insurance, and financial issues
· Providing member and their family/caregivers with educational material and coaching to
empower them to manage their health and well being
· Completing required paperwork and documentation within the timeframe required
· Collaboration with other professionals, disciplines, and participation in interdisciplinary
rounds
· Maintaining professional care management responsibilities by setting appropriate boundaries,
complying with CM Network expectations and productivity standards, and seeking managerial
consult as needed.
· Strategic and analytic thinking to address gaps in care.
